An affidavit (click here...), the proforma is displayed at the Consulate website, to be notarized and further to be attested by the Consulate along with the copy of the lost passport and then send all the documents over the email attest.toronto@mea.gov.in for the Consulate approval.
With the availability of copy of the lost passport, the applicant has to take pre-approval for submission of surrender application without the passport (as the passport is lost) at the BLS and on receiving the surrender application at the Consulate, the applicant will be issued Renunciation Certificate in place of Surrender Certificate by the Consulate for onward delivery by BLS to the applicant.
In case of absence of copy of the lost passport/earlier passport in both the above cases, then the applicant will be advised to apply for Nativity/Domicile Certificate.
For Nativity/Domicile Certificate, it should be issued by the office of District Magistrate/Deputy Collector/Tehsildar of the applicant's area in India. After obtaining the Nativity Certificate, it should be further attested by the Ministry of External Affairs, New Delhi.
Persons of Indian Origin (PIOs), who have acquired foreign citizenship are required to surrender their Indian passports to the nearest Indian Mission/Post immediately after acquisition of foreign citizenship.
After filling and submitting the form online ( https://passport.gov.in/nri/ ), applicant needs to submit the hard copy of form with photo, supporting documents and fee at BLS.

All the Surrender applications must be come through BLS. The Consulate will not accept any Surrender application directly from the applicant.

Indian passports of those acquiring foreign citizenship prior to 31 May, 2010 used to be cancelled and no Surrender Certificate was issued. In case they wish to apply for OCI card now, then they need to share the proof of cancelled passport over email to the Consulate on its email attest.toronto@mea.gov.in before submission of OCI application at BLS.
The concept of issue of Surrender Certificate came into existene on 1 June 2010. As you have surrendered your Indian passport in 2014, you must have received the surender certificate at that time.
In case you do not possess the surrender certificate as on date, you may send the scanned copy of your surrendered Indian passport for verification at this end.
It is mandatory to have surrender certificate after June 1, 2010 for applying for OCI.
For OCI application only applicant need to submit surrender certificate.
